A fifth grader labeled "a year behind" might be strong at geometry and measurement, fine at addition and subtraction, and genuinely lost on multiplication facts and fraction equivalence. Those last two then break everything downstream.
This distinction matters enormously for how much work is ahead of you. Re-teaching an entire grade is a year-long project. Repairing three specific skills is often a matter of weeks. Most families are facing the second situation while planning for the first.
You need the highest point where understanding is genuinely solid. Work backward, not forward.

Consider an evaluation if: focused work over three or more months produces no measurable movement; math is dramatically weaker than every other subject; your child can't estimate whether an answer is reasonable even after instruction; or basic number recognition and comparison remain shaky well past when peers were fluent. Dyscalculia is roughly as common as dyslexia and far less frequently identified, and children with it need specific instruction rather than more repetition.
Work backward. Start two grade levels below their current placement and sample four or five problems per skill, moving up one level at a time. Stop where accuracy drops below about 80% or where guessing begins. Also note whether errors are conceptual or just calculation slips — the fixes differ completely.
